Performance review form, provide score
Form to provide rankings (excellent, very good, good, needs improvement, poor) for each area (ex. Quality, Productivity/Independence/Reliability, Job Competency, Attendance, etc.) have numerical values associated with them. For example, if you were to rank someone "excellent" for the "quality" area, their quality score would be a "5". As you complete the performance review, the scores of each respective area would add up to get your "Final Performance Score". This number would assist supervisors in deciding if the employee deserves a raise or disciplinary action.
